I discovered that when you add a second head to the mount, that is when it disconnects you. you are then unable to click on it again after that. It will result in a disconnect with the warning that was posted within my video. hopefully that helps with troubleshooting the issue. (probably more of a facepunch issue is going to be my assumption) ~ Obsidian Knight

This might be a Facepunch issue or an issue with stack sizes for heads if heads become no longer stackable it will either fix it or confirm its a issue for facepunch to look into.
